Protected Health Information is individually identifiable health data that is created, stored, or transmitted by covered entities and is regulated under HIPAA in the United States. It includes diagnoses, treatment records, and payment information when tied to an identity. Safeguarding PHI drives requirements for encryption, access controls, audit logging, and breach notification. It is a specialized subset of PII, and mishandling it can trigger significant regulatory penalties, making data classification and least privilege essential controls in healthcare environments.
